A need for an improved system to support AAC users in the transition from paediatric to adult services has been identified in Oxfordshire. The paediatric and adult speech and language therapy services are separate departments that have limited contact. Although clients are routinely referred to the SLT team for Adults with Learning Disabilities, referral on to the AAC specialist has been ad hoc. Quite often the clients that are referred are no longer using their VOCAs effectively.

The existing method of service delivery will be described. Factors that have led to the abandonment of VOCA use will be discussed. The proposed changes to service delivery will be identified. Progress of these changes to date, and method of service delivery, will be described.
