Smoothing the Process of Transition from Special School to Adult Placement

This presentation will share our experience trying to improve the transition process from special school to college or adult placement.

• planning needs to be started well in advance
• setting up an SLT Transition Care Group
• liaison with careers advisors
• how we established stronger links with the Adult SLT team and also appointed one adult therapist for each of the special schools
• developing a Transition Pathway
• links with a local college
• providing appropriate vocabulary for each child’s AAC system for moving on from school.
